You’ll find client forms, day sheets, note-writing guides, and other resources designed to help you think clearly, stay aligned, and feel supported in the work you’re doing with clients.

More Than the Session

Sitting with a client and listening deeply is a vital part of therapy, and it is not the only part. Skillful clinical work also includes thoughtful documentation, ongoing learning, consultation, and supervision. These pieces work together to support ethical care, clinical clarity, and continuity over time. Documentation helps protect the client and the counselor, and this space offers forms and practical supports to help you write notes and create safety plans with clients.

Violet Portal

cultural competency trainings free from Medicaid/RMHP. We MUST complete trainings to keep our reimbursement rates up. You receive CEU credits and a certificate for each training completed. Currently, Top 3 Learners are: Jen A-K, Shawn & Lisa. Way to go!

BTM Forms

Please note:

All clients are required to complete forms directly through the client portal whenever possible. These PDFs are here only for rare situations when a client cannot access the portal and arrives to session without completed paperwork. To save time, paper, and a small forest , please do not routinely print these forms for clients.

 

That said, we do recommend printing a safety plan form whenever needed, as it can be helpful to complete together in session.
